Due to the current situation with Covid-19 and while the general public are being asked to stay at home, we are accepting online account applications by email.
You are required to read the information leaflet and mark ’Y’ in all boxes on the application form to indicate you have read and understood the information. Please download the forms and complete.
Please email your completed forms to firstname.lastname@example.org. Your account will be activated within 7 working days and you should receive a confirmation email.
We will contact you/ our patient by telephone to verify identification by asking questions relating to your medical record if you have emailed the application to us. Please make sure you include an up to date telephone number for the patient on the application form.
If you are bringing your form to the surgery you will need to provide two forms of identification, one to be photographic. For the full list of accepted identification please click on the identification document.
Proxy (Parents & Carers to complete with patient consent where applicable)
Please note: The person applying for proxy access to allow you to order repeat prescriptions etc on behalf of our patient is required to have their own online account already set up. The proxy access will be attached to your own online account. You will use your current username and password to log in.
All patients have the opportunity to register for SystmOnline. This allows you to:
- book and cancel appointments online
- order your repeat prescription
- view test results and your medical record.
You will need a unique username and password that can be obtained from the reception desk. You will need photographic identification.
