People with chronic diseases, or long term medical conditions often need regular monitoring and review. This is generally organised and led by the practice nurses. Most people will be invited for a review on an annual basis.
Where necessary, you will be invited for an appointment for blood tests and appropriate measurements (e.g. weight or blood pressure) a week or two ahead of your annual review. This way it is possible to consider and discuss the latest information at the clinic appointment.
